We recommend you start the process by completing our COVID-19 Patient Questionnaire. In the State of Illinois, laboratory tests are required to be ordered by an appropriate authorized person which includes, but is not limited to, physicians, nurse practitioners. Illinois has issued a waiver of this requirement for certain criteria that can be found here. However, if testing is obtained under this waiver, it is deemed a non-covered service and not authorized for reimbursement by a third-party payor.

Simple Laboratories recommends involving a provider in your care and we can assist you in receiving a telehealth consultation if necessary after you complete the COVID-19 Patient Questionnaire.

The Simple Laboratories team will then review and verify your information. You'll be contacted by either our scheduling team or by a telehealth provider who will have a consultation with you and authorize an order based on an independent medical-necessity determination.

More About our Tests

Simple Laboratories performs both PCR testing for the active infection and serology testing for COVID-19 antibodies (IgG & IgM).

PCR tests are used to directly detect the presence of a pathogen, rather than the presence of the body’s immune response, or antibodies. By detecting viral RNA, which will be present in the body before antibodies form or symptoms of the disease are present, the tests can tell whether or not someone has the virus very early on.

PCR testing may be helpful if you:

  • Are currently experiencing COVID-19 symptoms
  • Live in or recently traveled to an area with an ongoing spread of COVID-19
  • Have been in close contact with a person known to have COVID-19
  • Think you have been exposed to COVID-19

Antibody testing may help identify if you were previously exposed to the virus and, if exposed, can check whether or not your body has produced antibodies. Antibodies typically suggest protective immunity after you’ve recovered or been exposed to COVID-19. However, evidence is still being collected to determine if IgG and/or IgM antibodies provide protective immunity against SARS-CoV-2, the virus that causes COVID-19 infection.

Antibody testing may be right for you if you:

  • Have had a positive test for COVID-19 and it has been at least 3 days and you want to know if you have detectable levels of IgM and / or IgG antibodies
  • Have not experienced a fever or felt feverish in the last 3 days
  • Have not experienced new or worsening symptoms of COVID-19 in the past 10 days: loss of smell or taste, shortness of breath or difficulty breathing, feeling weak or lethargic, lightheadedness or dizziness, vomiting or diarrhea, slurred speech, and/or seizures

All COVID-19 testing at Simple Laboratories is performed on high-throughput instrumentation. Additionally, the tests have all undergone significant validation studies and are authorized under the FDA’s Emergency Use Authorization. We feel it is important to make a distinction between our testing versus the inaccurate rapid test kits that are drawing negative media attention.

Simple Laboratories offers several options of COVID-19 tests that identify if individuals are infected or may have been infected with coronavirus.

At Simple Laboratories, we can accept all patients that qualify for COVID-19 testing and can assist you in navigating how to properly apply the available benefits.  We have COVID-19 testing available with no out-of-pocket costs to the patient.  The Families First Coronavirus Response Act & CARES Act require that group health plans must provide coverage for approved COVID-19 testing without imposing any cost sharing (including deductibles, copayments and coinsurance) AND will also provide claims reimbursement for testing uninsured individuals for COVID-19.  For patients who prefer to access our self-pay options, please click here.
